FERPA and the Privacy of Student Records
Columbia College Chicago complies with the Family Educational Rights and Privacy Act of 1974 (FERPA or the Act), as amended, by publishing an annual notice to students of their rights under the Act in Columbia College Chicago’s Catalog and Student Handbook.

Release of Information
If you want to give your parents, or any other individuals, permission to have access to your academic and/or financial information, please print the appropriate form in the Resource Center.
To release information, select the Authorization to Release Information Form.
To revoke, select the Authorization to Revoke Release of Information Form.
The student needs to fill out the form completely and submit it in person to Columbia Central with a valid student ID. Completed forms can also be scanned and emailed to columbiacentral@colum.edu, but will only be accepted if sent from the student's official college email.
Please be aware that this release is for inquiries and discussion purposes only. The college will not automatically mail grades, transcripts, or any financial information to the individuals authorized on the form. If you want your enrollment status sent to a lender, this is not the correct form, please complete the Request for Verification Form.
Disclosure Refusal
If you do not want any information released regarding your enrollment status or educational history, you may fill out a Disclosure Refusal Form in the Resource Center. Disclosure Refusal is available only for currently enrolled students.
