Dependency Information

If you answered "yes" to certain dependency questions on your FAFSA, you will also need to provide documents to verify these data elements entered on your FAFSA. 

If you must submit dependency documentation, you will receive an email to your student email. 

View your FAFSA Subumission Summary online at studentaid.gov by clicking on "View Processed Information."

If you answered yes to any of the below questions, you must submit the corresponding documentation. 

If, after viewing your FAFSA Submission Summary, you determine that you answered yes to any of these questions in error, you should make a correction to your FAFSA Submission Summary and change your answer to no. 

If you are unable to provide documentation for any of these questions, or if you are unable to provide documentation but feel you have extenuating circumstances (abusive or unsafe home environment, homeless or at risk of homelessness without an official determination, etc.), please call us at 312-369-7140 to speak with a representative.

2024-25

 

  • Question 5 Student Personal Circumstances
    • The student is currently severing on active duty in the U.S armed forces for purposes other than training OR
    • The student is a veteran of the U.S armed forces OR
    • At any time since the student turned 13, they were an orphan (no lining of biological or adoptive parent) OR
    • At any time since the student turned 13, they were a ward of the court OR
    • At any time since the student turned 13, they were in foster care OR
    • The student is or was legally emancipated minor, as determined by a court in their state of residency OR
    • The student is or was in legal guardianship with someone other than their parent or stepparent, as determined by a court in their state of residence
  • Question 6 Student Other Circumstance

    At any time on or after July 1, 2023, was the student unaccompanied and either (1) homeless or (2) self-supporting and at the risk of being homeless? If the answer is "Yes" please provide one of the following:

    • Submit a singed letter on official letterhead form your high school or school district homeless liaison determining you to be an unaccompanied youth who was homeless on or after July 1, 2023 OR
    • Submit a signed letter on official letterhead form the director of an emergency or transitional housing program funded by the U.S. Department of housing and Urban Development determining you to be an unaccompanied youth who homeless on or after July 1, 2023 OR
    • Submit a signed letter on official letterhead form the director of a runaway or a homeless youth center or transitional living program determining you to be homeless or self-supporting and at risk of being homeless on or after  July 1, 2023.

     

     

     

  • Question 8 Apply for Direct Unsubsidized Only

    Are the student's parent unwilling to provide their information, but the student does not have an unusual circumstance that prevents them form contacting the parents or obtaining their information? If the answer is "Yes" please provide the following: 

     

    • A signed and dated statement from your parents of the refusal to complete the FAFSA OR
    • A signed and dated statement from your parents unwilling to provide any financial support including the date support ended

     

    If approved student will not qualify to receive other types of federal student loans (including Direct Subsidized Loans), federal grants or Federal Work-Study programs.
